Solo 401k Application

Terms:

Monarch IRA, LLC and/or its designees will prepare and provide to you all necessary Solo 401k plan and trust documents to give you checkbook control of your funds as Solo 401k trustee. Plan documents include an IRS determination letter proving the plan’s qualified status. We will also obtain or assist you in obtaining an EIN for your new 401k trust. This EIN is different from any EIN or taxpayer ID you may already have for your business. We will provide annual maintenance including any amendments or restatements required to keep the plan’s qualified status with the IRS. Annual maintenance is priced at $200. The first annual fee is due starting 1 year after the first day of the month in which the application was submitted and will continue each year thereafter. Late fees may apply to annual fees not received before the end of the billing grace period. A $50 termination fee applies for plan termination or cancellation of ongoing maintenance and support.

Our services are intended for an individual who is a fiduciary to his or her own 401k. You hereby recognize that as a plan fiduciary, you are responsible for making decisions about the investments of the plan as well as when and where to seek advice and services for the plan. Our services do not transfer any fiduciary responsibility to Monarch IRA, LLC and/or its designees.
